Care Coordination for Children (CC4C)

What is CC4C?

Care Coordination for Children (CC4C) is a free community service for special needs children age 0-5 and their families.  CC4C strives to ensure that children are raised in a healthy, safe, and nurturing environment so that they can reach their full potential. 

What does CC4C Provide?

  • CC4C helps parents schedule appointments, communicate with doctors, and track well-child care and immunizations.
  • CC4C identifies and coordinates services and resources to help meet the family’s specific needs such as housing, medical care, transportation, child care, financial aid, and counseling/support programs.
  • CC4C educates parents about nutrition, well-child care, school health, genetic services, newborn screenings, and various resources.
  • CC4C provides ongoing encouragement, support, and tools to strengthen families.
  • CC4C reduces toxic stress for children.
  • CC4C works with medical homes to provide smooth transition across care settings.

Who is Eligible for CC4C?

Eligibility is not based on finances, but on risk factors and diagnoses.  Parents can call to see if they are eligible, or be referred by doctors, hospitals, social workers, family members, day care providers, or other state/county agencies.  For more information, call 828-452-6675.
